Usually this means that emblem is set in BF1 or BFV and it "glitches" over only in the kill cam

Not a glitch. Emblems are set in the Battlefield Companion mobile app. That goes for BF4, BF1, and BFV. This is why you can set a totally different emblem in Battlelog and it won't show in game.
