These screenshots are from the same frame during two different playbacks of a demo. Notice how different the smoke is? This is why sometimes one player can see through smoke while another can't.

Also I'm pretty sure the kind of hack you're talking about isn't even possible since the server is authoritative on things like ammo and equipment. Even the most obvious speed hacking aimbotter has to reload and can run out of grenades.
